About the award
Every year, in honor of National Doctors Day, we celebrate the incredible work of our physicians and thank them for their dedication to healing body, mind and spirit by awarding one exemplary physician with the Alfred Valles, MD Legacy Award.
This annual physician award – formerly known as Champion in Care – was renamed in 2024 to honor the life and legacy of Alfred Valles, MD, a beloved physician, mentor and friend in the Saint Agnes community who passed away tragically in October 2023.
The award is given to one physician annually who demonstrates a team spirit and desire to work collaboratively to provide exemplary patient care.
Nomination criteria
- Recognizes the value of individual and shared knowledge when interacting with others.
- Demonstrates respect for all members of the care team and is open to different points of view.
- Communicates openly and effectively.
- Supports others in engaging patients as active participants in their care.
- Appreciates the integral role teamwork plays in the delivery of care and in improving patient outcomes.
Submit a nomination
All are welcome to submit a nomination for the Alfred Valles, MD Legacy Award. Simply complete the form below with the required information to nominate a physician.
Nominations will only be accepted for physicians who are active members of the Saint Agnes Medical Staff and in good-standing. One physician will be selected through a blinded judging process. Nominations must be received by Friday, March 14, to be considered.
