Affiliation Letter Request

If you are in need of an affiliation letter for a Saint Agnes Physician or Allied Health Practitioner, please send a request and signed release via e-mail to SAMCMedicalStaff@samc.com or via fax to (559) 450-3370. Once we receive the signed release, you will receive the link to our affiliation site within 5 business days.
